Structured Clinical Interview for DSM-IV-TR Axis II Personality Disorders (SCID-II)

The SCID-II is a version of the SCID developed for the assessment of DSM-IV-TR Personality Disorders. It is published by American Psychiatric Publishing, Inc. and can be ordered through the APPI web site or by phone 800-368-5777.  Unlike the SCID-I, there is only a single version of the SCID-II (i.e., there are no separate research and clinician versions).

There are three components to the SCID-II.

The interview itself covers the eleven DSM-IV Personality Disorders (including Personality Disorder NOS) and the appendix categories Depressive Personality Disorder and Passive-Aggressive Personality Disorder.

The SCID-II Personality Questionnaire is available as a screening tool to shorten the time it takes the clinician to administer the SCID-II. After the subject fills out the Personality Questionnaire (which usually takes 20 minutes), the clinician simply circles the numbers to the left of the SCID-II items that correspond to items answered "yes" on the questionnaire. When the SCID-II is administered, the clinician needs only to inquire about the items screened positive on the questionnaire. The assumption is that a subject who responds with a "no" on the questionnaire item would also have answered "no" to the same question had it been read aloud by the interviewer.

The 100+ page SCID-II User's Guide contains several particularly notable features. In addition to providing detailed instructions and guidelines on how to effectively administer the SCID-II, the User's Guide contains the following:

  • an item-by-item commentary covering issues to consider in assessing each of the DSM-IV Personality Disorder items
  • a case example (taken from the DSM-IV Casebook) plus the corresponding SCID-II that has been completed
  • a discussion of how to apply the DSM-IV general diagnostic criteria for Personality Disorders (DSM-IV, page 633) to making SCID-II ratings.

In order to master how to administer the SCID-II, we recommend that you study the instructions and guidelines in the SCID-II User’s Guide and then practice doing the SCID-II on colleagues or friends/relatives in order to become familiar with the reading of the questions. To check your ratings against a reference interview, you can purchase the "Supermom II" DVD. Finally, a computer-assisted version of the SCID-II, called the CAS-II, has been developed by MHS and is available from their web site (www.mhs.com). It also comes with a computer-administered version of the SCID-II-PQ (Personality Questionnaire). Like the SCID-II, the CAS-II can be set up so that, "no" answers on the computer-administered SCID-II-PQ will cause the CAS-II to skip the corresponding interview questions.
